Pustulosis exantematosa generalizada aguda

La pustulosis exantematosa generalizada aguda (PEGA) es una patología dermatológica poco frecuente, caracterizada por la aparición brusca y de múltiples pústulas puntiformes, estériles, no foliculares, sobre base eritematosa edematosa. Es desencadenada frecuentemente fármacos, entre los cuales resaltan antibióticos. Presentamos el caso un paciente varón 40 años, que ingresó a emergencia síndrome doloroso abdominal descartar tumoral intraabdominal, indicándole inicialmente ceftriaxona metronidazol. Al día siguiente, reingresó encontrando, tras cirugía emergencia, apendicitis complicada con perforación intestinal, lo cual rotan antibioticoterapia imipenem-cilastatina + A las siguientes 48 horas, presentó eritema generalizado algunas pequeñas microvesículas en región torácica, extremidades superiores cuello. Asimismo, se observó exámenes laboratorio, leucocitosis neutrofílica e hipoalbuminemia. En biopsia, encontró neutrofílicas subcorneales intraepidérmicas, dermis papilar edematosa infiltrado inflamatorio perivascular presencia neutrófilos escasos eosinófilos. Con todo descrito anteriormente, llegamos conclusión PEGA, desencadenado antibióticos recibidos, o imipenem-cilastatina. Tras 6 días suspensión imipenem-cilastatina, mostró mejoría lesiones dérmicas, leve escasa descamación fina.
